The Creticos Cancer Center is the hospital-based outpatient cancer center for Advocate Illinois Masonic. Located in the heart of Wrigelyville, the center is nationally accredited by the American College of Surgeons Commission on Cancer for high quality care. The purpose of this position is to develop a comprehensive survivorship program in collaboration with physicians and other members of the team. The coordinator will maintain ongoing relationships and act as a liason with health care providers and members of the community (e.g. Psychosocial Oncology, senior communities, American Cancer Society, Gilda's Club). In addition, the incumbent is responsible for creating survivorship careplans for assigned patients and meeting with patients per program guidelines. The incumbent will report outcomes data to Cancer Committee and Cancer Institute leadership.

BSN ~3~5 years working with Oncology patients. ~OCN certification preferred ~Good communication skills. ~Knowledge of different computer databases for patient information ~Knowledgeable of healthcare industry ~Excellent written and verbal communication skills CPR certification OCN, preferred Ability to work under stressful conditions and in difficult situations. Ability to work with patients and family members in a fast~paced environment with established time constraints. Must demonstrate empathy and compassion with all patient and family interactions. Must be willing to work with other Advocate sites.
