Clinical Program Coordinator, RN- Breast Cancer

The Navigation Clinical Program Coordinator (NCPC) is responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services – from initial referral to follow-up care. In conjunction with the LIP, provide necessary education and information on available options, the informed decision making process, and realistic goal setting in order to empower the patient/caregiver to actively participate in their plan of care. Works in concert with the Patient Advisory Council to coordinate, implement, and evaluate educational programs/projects for patients and the community at large. The NCPC adheres to the Nursing Standard of Care/ Practice, the Plan for Provision of Nursing Care, the Plan for Provision of Care to Patients, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan.
The candidate must have a Bachelor’s degree of Science in Nursing, a current California Registered Nurse License, and 3-5 years Oncology experience. Oncology Certification is highly recommended and will be required within one year of hire.
