The University of Minnesota Health Cancer Care located in Wyoming, MN has an RN Case Manager position available. The Case Manager plans, organizes, coordinates, and controls a caseload of patients and may serve as lead by providing coordination of department activities.

Location: Wyoming, MN
Serves as a resource and educator to patients, medical and hospital staff. Works as an extension of and partners with physicians and providers to coordinate care efforts for a safe, effective, efficient and patient-centered transition along the health continuum. Provides pro-active disease management using evidence-based practices. Acts as a liaison between patient/family, medical staff and others involved in treatment, promoting patient-centered care based on working jointly with their assigned physicians to reach the desired health care goals and priorities for the hematology/oncology patient population.
Six months clinical experience
Current/active MN RN License in good standing
Oncology Nurse Certification (OCN) when criteria are met for eligibility period of the ONS requirements.
Bachelor Degree in Nursing or health related field
