The Oncology Nurse Navigator (ONN) is a Registered Nurse with comprehensive oncology knowledge who demonstrates clinical judgement, assesses the needs of patients and their families/caregivers in a holistic approach, and provides care coordination, education and support throughout the disease process and cancer care continuum. The ONN serves as central point of contact after diagnosis and helps Legacy Cancer Institute (LCI) patients and their families navigate the health care system in an organized, effective and efficient manner. The ONN provides expert guidance to patients and families to improve their understanding, prepare them for what to expect, and promote their participation in the cancer care and informed decision-making process. The ONN facilitates the timely flow of information between the various clinicians involved in the care of the patient and promotes communication between patients, families/caregivers and providers to improve quality, timeliness and continuity of care.
Must be able to travel between all Legacy Health medical centers, collaborative community sites, off-campus professional development events and community outreach activities using personal and/or public transportation.
Initiates contact with patients upon referral and at anticipated high stress/pivotal points throughout the cancer care experience (e.g. time of diagnosis, changes in treatment plan, transitions in care, post-treatment survivorship); makes initial and ongoing assessments of patient needs and provides appropriate interventions
Serves as patient advocate; provides direct and indirect patient care (in person and by phone) throughout the continuum of care and into survivorship
Assesses educational needs; provides appropriate and timely education to patients and family members about the cancer diagnosis/disease process, cancer care team roles, treatment options, plan of care and available resources/support services (institutional and community)
Works closely with interdisciplinary teams; builds and maintains collaborative relationships with physicians, team members and departmental staff to facilitate timely care, improve communication, and promote continuity of cancer care services within and across systems
Identifies and assesses potential and realized barriers to care; facilitates referrals and access to services as appropriate to mitigate barriers and expedite the plan of care
Stays current on evidence-based practices and trends in oncology nurse navigation; takes responsibility to pursue professional growth and maintain education/skills/competencies
Takes part in patient/public/professional cancer-related education and community outreach event
The ideal candidate will have excellent communication and interpersonal skills, demonstrated expertise in area of oncology specialty, navigation/care coordination experience (especially in the ambulatory care setting) and desire to work with interdisciplinary cancer care team(s). Attention to detail and strong critical thinking ability is a necessary quality as well as passion for oncology patient care and cancer program development.
BSN or actively matriculated into a BSN degree program with the expectation that BSN degree will be completed within 6 months of hire
Minimum two years of relevant adult oncology experience
Excellent communication skills, both written and oral; able to adjust to variable audiences and levels of education; strong interpersonal skills
Demonstrates a caring compassionate attitude toward others; cooperative and approachable demeanor; readily acts as a role model and staff resource
Requires minimal supervision; self-motivated; able to work both independently and collaboratively
Strong problem-solving, organization/time management and prioritization skills
Demonstrates professionalism; remains composed under stress; follows through on commitments
Computer proficiency to include: EMR (EPIC), Microsoft Office and Web-Based Applications
Master’s degree in nursing
OCN certification, must be obtained within 2 years of hire. Additional certifications related to oncology nurse navigation/area of oncology specialty recommended
Care coordination/nurse navigation experience
Accreditation and/or program development experience
