Hem/Onc Nurse Navigator Care New England Warwick, RI
Care New England continues to advance its hematology/oncology services by expanding and advancing its community service locations. Our long-standing practice of three hem/onc physicians and a nurse practitioner is adding a nurse navigator to enrich the care management of its patients.
The nurse navigator will:
Assist patients and their families in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and available treatment therapies.
Serve as a resource for community support group collaboration.
Assist providers and patients to identify benefits and coordinate resources based upon patient’s needs, insurance coverage and community programs.
Serve as an essential link between patients and all other care providers.
Facilitate appointments for consults and support services, including nutritionist, oncologist, physical therapist, etc.
Attend weekly oncology clinical research meetings, tumor boards to stay up to date regarding the patient care.
Licensed in the state of RI.
BSN plus 5 years of nursing experience in oncology.
Oncology Nursing Society certification required.
Nurse Navigation Certification Preferred and/or obtained within 6 months of hire with tuition reimbursement support.
A sign-on bonus of $4,000 is available to the finalist.
