You Will Be Responsible For:
Responsible for the supervision of Head and Neck Care Coordinators and Head and Neck data abstractors. Manages staff performance by follow-up on accountabilities, individual meetings, counseling, and evaluation.
Facilitates an evidence-based, multidisciplinary, multi-specialty approach to cancer care throughout the care continuum.
Remains current in evidence-based cancer practice and new emerging trends in area of cancer specialty.
Manages the Head and Neck Program Leadership Committee including identifying and developing strategic program initiatives with the Head and Neck physician leadership team and FHCI administration. The committee promotes and evaluates compliance to established national Head and Neck guidelines in the care of patients with Head and Neck cancers to improve clinical outcomes, participation in clinical trials and patient satisfaction. In addition, the committee is a platform for the reporting and publication of clinical outcome results, promoting the educational and research mission and making recommendations for new program services and technology.
Promotes Coordinated Cancer Program awareness within the physician community and the community at-large. Care Coordinator represents AHCI in the community as the “Cancer Ambassador” for designated tumor site. Serves as a consultant to AH affiliate hospitals (within the 17-hospital system) requesting support for development of Head and Neck Cancer Programs.
Develops appropriate relationships to maximize Care Coordinated Program effectiveness and facilitates program problem resolution across departmental, campus, and physician/provider lines.
Navigates a diagnostic or cancer patient (needs ranging for minimal to complex) from the diagnostic phase through treatment planning, medical management, and surveillance/survivorship ensuring the patients have appropriate, timely, medical, supportive services through a multidisciplinary approach to patient care. Addresses appointment coordination, record requests, insurance concerns, education, and communicating information between the physician office and the patient, etc.
Participates and supports the Tumor Boards, Journal Clubs, and other physician education venues. Supports AHCI Corporate Relations Department regarding these physician programs and assists as requested with program activities. Communicates Tumor Board recommendations to all involved physicians (including primary care physicians) and other health care providers.
Co-facilitates patient/family support and education programs providing both one-on-one sessions and group forums.
WHAT YOU WILL NEED:
KNOWLEDGE AND SKILLS PREFERRED:
Proficient in Microsoft Office (Word, Outlook, Excel, Power Point, Access)
EDUCATION AND EXPERIENCE REQUIRED:
Three to five years’ experience as a Head and Neck Care Coordinator and/or Supervisor
Five to seven years’ experience in oncology nursing practice or in Head and Neck care/Head and Neck cancer nursing practice
Five to seven years’ management experience
EDUCATION AND EXPERIENCE PREFERRED:
Bachelor’s or Master’s degree in Nursing
LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:
Current active State of Florida license as a Registered Nurse
Basic Life Support (BLS) Certification
LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED:
National Certification: Oncology Certified Nurse (OCN) through the Oncology Nursing Certification Corporation
The overall goal of the Care Coordinator Manager/Head and Neck (CCM/HN) position is to facilitate an evidence-based, multidisciplinary, multi-specialty approach to cancer care throughout the care continuum. The Care Coordinator Manager/Head and Neck develops, manages, and redesigns, as needed, Multidisciplinary Head and Neck Clinics accommodating Head and Neck cancer patients with multiple physician consultations within a half-day period. Co-responsibilities for all clinic components including patient registration, clinic charges, medical records, films, pathology slides, patient health history, pre- and post-clinic discussions, presentation of clinic patients at the Head and Neck Tumor Boards, and follow-up letters communicating treatment recommendations sent to the physician team and patient. Patient needs may range from minimal to complex and may occur in the inpatient/outpatient arena. The CCM/HN gathers, records, and evaluates program data and uses this data in establishing program goals in collaboration with AHCI Outcomes and Values Program. The CCM/HN supervises both the care process and employee(s). This position is accountable for the ongoing monitoring of program quality and supports all components of program marketing.
