This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Experience with high volumes of workload (patient census) with demonstrated flexibility, professionalism in support of patient care standards. Experience with educating patients, families, and caregivers on the diagnostic and staging process as well as current emerging treatment options available. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. The Registered Nurse, Oncology Care Navigator (OCN) supports patients and their families through the continuum of cancer care. The incumbent is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. The incumbent will develop and execute appropriate multi-disciplinary oncology navigation activities in collaboration with the Chief of Staff (COS), Associate Director, Patient Care Service (ADPCS), Deputy ADPCS, RN Disease Manager (DM), Group Practice Manager (GPM), Patient Aligned Care Teams (PACT), Community Care Services (CCS) and others as needed in support of the Veteran's health needs. The incumbent will complete an annual strategic analytics to identify, assess, and prioritize the needs of the Veteran population as it relates to oncology care. Uses data-driven processes to prospectively and retrospectively identify indicators of ineffective or inefficient delivery of care and make recommendation for improvement. The OCN is responsible for outreach efforts to establish and maintain positive working relationships with key community stakeholders (e.g., community hospitals and providers, etc.). The incumbent participates in quantitative and qualitative data collection efforts as needed to support program evaluation and quality improvements. The OCN provides education/training to all clinicians and services on the OCN's role and processes. Serves as a single point of contact and resource for oncology care navigation ensuring continued communication with the VA Healthcare system, the Veteran, and Patient Aligned Care Team (PACT), the community provider(s) and hospitals, etc. The incumbent assists Veterans in the transition through the continuum of care, in understanding processes regarding access to care, disease process, next steps, and overcoming barriers to care to include eligibility, referral initiation/renewal, and flow of care between the VA and community care, etc. The OCN serves as an essential link and bridge between Veterans, community providers, VA providers, PACT, CCS, and services to ensure Veterans receive the services and care needed. Will ensure a warm handoff (person-to-person verbal communication to provide continuity of care and a seamless transfer of information) for Veterans in transition from community care back to WTVAHCS PACT. The OCN documents the warm handoff in the Computerized Veteran Record System (CPRS). Additionally, the incumbent documents all Veteran interactions/encounters/communication using face-to-face, Clinical Video Telehealth (CVT), telephonic, VA Video Connect (VVC), or secure message (SM) in CPRS. Will appropriately code encounters/communication/historical notes using the International Classification of Diseases (ICD), Evaluation and Management (E&M), and DM-specific Healthcare Common Procedure Coding System (HCPCS) codes. Encounters will be completed and signed within 72 hours of initiation. The incumbent conducts nursing peer reviews as assigned. The OCN is a member of the Health Promotion and Disease Prevention (HPDP) Program Committee and actively participates in care coordination meetings/committees as assigned.
