The Nurse Navigator is responsible for the coordination of newly diagnosed cancer patients, acting as a liaison between patients, families, community, and physicians. The Nurse Navigator is accountable for coordinating care for patients as part of a multidisciplinary team of physicians and/or ancillary department personnel involved with cancer patients at MCI and served by the MemorialCare Medical Group . The Nurse Navigator collaborates with other disciplines to provide safe and effective patient care delivery by triaging and prioritizing patient needs and availability. The achievement of [desired] quality patient care is accomplished through the nurse navigation process and evidence-based practice for quality outcomes throughout the continuum in alignment with the strategic plan.
- Demonstrates knowledge of clinical guidelines in cancer care (National Comprehensive Cancer Network, National Cancer Institute, Commission on Cancer, Oncology Nurses Society). Present at multidisciplinary committee meetings.
- Utilizes the nursing process as the framework for the management of patient care.
- Utilizes the ONS guidelines in the management of patient care.
- Demonstrates leadership in the coordination of patient care across the healthcare continuum.
- Prioritizes essential functions through all phases of care including prevention, detection, diagnosis, treatment, research, and survivorship.
- Delegates to licensed and unlicensed personnel.
- Communicates effectively and succinctly with the entire healthcare team.
