Responsible for providing guidance and coordination of care for patients and their families through the process of diagnosis and treatment. Responsibilities also include support of the oncology program and providing direct patient care.Â
ESSENTIAL DUTIES:
The percentage of time spent performing essential functions is 95%. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:Â
Act as Liaison between physician and patient.Â
Serves as a resource for medication specialists and pre-certification associates to provide information which prevents treatment denials resulting in required peer-to-peer scenarios.
Acts as a resource for new patient schedulers, assisting to prioritize patients for scheduling and insuring that adequate information has been received from the referring provider.
Coordinates and provides patient education as indicated.
Participates in programs including marketing and community outreach. Maintains relationship with extramural organizations and manufacturers regarding the latest advancements in standards of practice, policies and treatment drugs and devices.
Serves as a resource to the nursing team to provide continuity of patient care.
Assures quality care is delivered as prescribed by the attending physician, regulatory guidelines, and department policies and procedures.
Provides leadership in quality performance and patient satisfaction.
Participates in orientation of new personnel to department, including policies and procedures,
Reviews ER visits and hospital admissions daily.
Facilitate a support group as indicated.
Actively participate in medical group committees as assigned.
Collaborates in development and implementation of clinical practice policies and programs for the delivery of specific patient care services, in collaboration with the clinic's medical director and the practice management team which may include standards of practice, and patient outcome evaluations.
Responsible for mentoring nursing and other personnel in assigned area.
Ensures continuity of care between providers within the PSH system and outside the system.Â
NON-ESSENTIAL DUTIES- The percentage of time spent performing secondary functions is 5%. The following duties are considered secondary to the essential duties listed above:Â
Performs other duties as assigned.Â
MINIMUM QUALIFICATION(S):
Graduate of an accredited school of nursing required;
Bachelor of Science degree.
Minimum of three (3) years nursing experience.Â
Current RN licensure in the state of employment.Â
BLS certification required.Â
