The role you’ll contribute:
The Care Coordinator Manager/Head and Neck & Generalist (CCM HN & G) facilitates an evidence-based multidisciplinary, multi-specialty approach to cancer care throughout the care continuum.Supports the development of and delivery of navigation pathways reflecting nationally recognized clinical practice guidelines.Patient needs may range from minimal to complex and may occur in the inpatient/outpatient arena.Works in collaboration with the Physician Team, the identified AHCI Subcommittees, the Department of “Institute Outcome and Value”, and the Administrative Team in establishing program goals.Share accountability for ongoing monitoring of program quality including accreditation, leadership committees and programs marketing initiatives.

Navigates Head and Neck Cancer patients identified throughout the system and also navigates other cancer diagnosis identified in the CFD-South Region (needs ranging from minimal to complex) in providing an evidence-based, multidisciplinary, multispecialty approach to cancer care throughout the care continuum.
Remains current in evidence-based cancer practice and new emerging trends in the area of cancer specialty.
Supports the development and delivery of navigation pathways reflecting nationally recognized clinical practice guidelines; strategizing to incorporate/consider insight from the Physician Team, Administration, Department of “Institute Outcome and Value” and AHCI subcommittees.
Responsible for the supervision of Head and Neck Cancer Care Coordinators.Manages staff performance by follow-up on accountabilities, individual meetings, counseling, and evaluation.
Participates in the ongoing development/launch of the Head and Neck Oncology Subcommittee in collaboration with the department of “Institute Outcome and Value”.
Promotes Navigation Program awareness within the physician community and the community at large.
Participates and supports Tumor Boards, Journal Clubs, and other physician education venues.Supports AHCI Corporate Relations Department regarding physician programs and assist as requested with program activities; i.e. cancer screening programs.
Develops appropriate relationship to maximize patient navigation effectiveness and facilities program problem solving across departmental, campus, and physician/provider lines.
Qualifications

KNOWLEDGE AND SKILLS REQUIRED:
·Team-centered leadership and facilitation skills; ability to interact appropriately with department/teams to facilitate process development, quality improvement, and interaction with patients/families and the Coordinated Care Program Team
·Strong organization, communication, and customer relations skills; ability to communicate effectively with diverse populations
·Skilled in providing age-appropriate cancer education
·Skilled and comfortable in discussing cancer diagnosis with patient and family.
·Flexibility in prioritization, ability to multitask, and critical thinking/problem solving skills.
·Self-motivated and ability to work independently.
KNOWLEDGE AND SKILLS PREFERRED:
·Data analysis
·Program development
·Proficient in Microsoft Office (Word, Outlook, Excel, PowerPoint, Access)
EDUCATION AND EXPERIENCE REQUIRED:
Five years’ experience in oncology nursing practice.
Five years’ management experience
EDUCATION AND EXPERIENCE PREFERRED:
·Bachelor’s or Master’s degree in nursing
·Three to five years’ experience as a Head and Neck Care Coordinator and/or Supervisor
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Current valid State of Florida license as a Registered Nurse
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
·National Certification: Oncology Certified Nurse (OCN) through the Oncology Nursing Certification Corporation
·Certified Otorhinolaryngology Nurse (CORLN)
