Under general supervision, provides professional nursing care for patients adhering to national and company standards and guidelines for specialty regulations. Demonstrates knowledge of physiological, psychological, and developmental and age specific differences in the patient population including late adolescence, adulthood, middle age, and the elderly. Collaborates with the interdisciplinary team to provide for assessment and therapeutic management of patient/family needs. Supports and adheres to the US Oncology Compliance Program, to include the Code of Ethics and Business Standards.
Salary Range: $36.00-$44.00

Responsibilities: The Diagnostics RN Navigator is responsible for the coordination of care for patients receiving Theranostic (nuclear) modalities for specific tumors, to include case coordination in collaboration with the diagnostics department, pre procedure checklists, patient teaching, lab result interpretation and follow up, direct patient care during procedures as required, post procedure tracking with case metrics.
Routine collaboration with internal departments and external referral resources are anticipated, including scheduling, financial counseling, lab, pharmacy, social work, nursing, surgical, radiation and imaging. Regular and frequent interface with practice-based leadership teams, physicians, and clinical staff is required to promote and engage the patient navigation process.
Key Responsibilities
Performs coordination of care pre and post procedure(s)
Develops appropriate nursing diagnoses and corresponding action plans as needed
Completes patient assessment and facilitates monitoring as warranted pre, during, and post procedure
Identifies any potential gaps in care delivery to optimize the patient experience and promote best care outcomes
Provides treatment and procedure education to the patient, caregiver, family or significant other to include side effect monitoring and any nuclear modality precautions
Holds or participates in weekly diagnostics/theragnostic huddles or team communication
Contributes to research, quality, and performance improvement metrics
Supports patient advocacy, empowerment, and protection of PHI Qualifications: Minimum Requirements:
Bachelor's prepared Registered Nurse (BSN), Oncology Certification (OCN) preferred, Nurse Navigator experience a plus
5 (five) years' work experience as a Registered Nurse
Critical Skills:
Ability to multitask & prioritize effectively, quick decision making, dedicated to best practice principles apply the full nursing process in day-to-day clinical operations, including assessment, analysis and nursing diagnosis, care planning, implementation, and outcomes evaluation, identify specific needs of care populations and cultures found within the general facility service area, strong organizational and critical thinking skills, excellent communication skills both verbal & written.
Proficient in computer use to include general software programs (Microsoft word, Chrome, Teams), Healthcare EMR review and documentation, Excel and/or Smartsheet spreadsheets for data & metrics collection
Basic knowledge of local community resources
