The Traveling Veteran Coordinator (TVC) utilizes an interdisciplinary approach to coordinate care required by patients with chronic, catastrophic, or complex high-risk or high-cost health issues who receive care throughout the country. The TVC develops and coordinates the seamless transitioning of patients throughout the VA system for his or her inpatient and outpatient care process with a focus on care coordination and case management needs Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
