We are seeking a patient focused, compassionate Nurse Practitioner or Physician Assistant to join our interdisciplinary team in Palliative Care! Position Details:Full-time, Day Shift No call, no weekendsOutpatient - visit patients in their homesKey Responsibilities include seeing patients, leading family meetings, writing orders and notes. Serve those most in need by managing symptoms and determine goals for care.Patient Volume: 4-5 new patients per week, as well as follow-up visits with existing patientsQualifications:Master's Degree requiredACNP, AGNP, FNP or PA Certification required
