Cancer Care Coordinator for Hematology/Oncology-Nurse Practitioner
The Nurse Practitioner provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding. GENERAL DESCRIPTION: The primary focus of practice is coordination of care for patients in the Hematology/Oncology Section including patients needing chemoradiotherapy, stem cell transplantation, chemotherapy, telehealth visits and other care at Durham VAMC, other VA facilities or in the community.
